Communicating an unfavourable prognosis while maintaining patient hope represents a critical challenge for healthcare professionals. Duty requires respect for the right to patient autonomy while at the same time not doing harm by causing hopelessness and demoralisation. In some cases, the need for therapeutic privilege is discussed. The primary objectives of this study were to explore HPs’ perceptions of hope in the prognosis communication and investigate how they interpret and operationalise key ethical principles. Sixteen qualitative semistructured interviews with HPs from (...) different positions and experience, including doctors and nurses in four different departments, were conducted in the Ticino Cantonal Hospital, Switzerland. The interviews were recorded, transcribed verbatim and analysed using thematic analysis. HPs defined prognosis and patient hope as interdependent concepts related to future perspectives for subjective quality of life. Two main factors allow HPs to maximise the benefits and minimise the harm of their communication: respecting the patient’s timing and sharing the patient’s wishes. Time is required to reframe needs and expectations. Furthermore, communication needs to be shared by HPs, patients and their relatives to build common awareness and promote a person-centred approach to prognosis. In this process, interprofessional collaboration is key: doctors and nurses are complementary and can together guarantee that patients and relatives receive information in the most appropriate form when they need it. Organisational aspects and the HPs’ emotional difficulties, particularly in coping with their own despair, are barriers to effective communication that need further investigation. (shrink)

L’obiettivo che questo lavoro si propone è quello di rintracciare nella prosa appassionante e appassionata di Michele Federico Sciacca tracce che rimandano alla speculazione filosofica di Bernardo di Chiaravalle. Divisi nel tempo, ma compartecipi di un modo comune di intendere la categoria mistica, i due filosofi appaiono uniti nella condivisione di una stessa sensibilità. È una filiazione che accoglie più interrogativi che certezze, primo tra tutti, il perché Sciacca non citi mai direttamente Bernardo, cosa che invece ripetutamente fa (...) con Agostino, Tommaso, Bonaventura, e altre colonne portanti del pensiero. Sono molte in tal senso le ipotesi che vengono prese in considerazione, tra queste l’idea che per il filosofo di Giarre, Bernardo fosse non una semplice fonte, ma una lente attraverso la quale guardare meglio e più lontano. Un legame taciuto dunque, tenuto a riparo dalla vista dei più, quello che Sciacca ha lasciato crescere tra le pieghe dei suoi scritti. I due filosofi sfidano il tempo e si trovano, o ri-trovano dentro una sincronicità filosofica che li vede messaggeri dagli stessi valori. In questo forse “immaginario” dialogo tra un pensatore che ha saputo sporgere lo sguardo oltre il suo secolo, e uno che non ha mai perso occasione per riflettere e riflettersi attraverso il passato, è possibile ritrovare e godere della tensione emotiva dell’essere umano che pretende di ricongiungersi con l’Assoluto. The aim of this paper is to find in Michele Federico Sciacca’s fascinating and passionate prose traces that refer to Bernard of Clairvaux philosophical speculation. Distant in time, but sharing a common way of understanding the mystical category, the two philosophers appear united in having the same sensitivity. It is a filiation that welcomes more questions than certainties. First of all, why Sciacca never quotes Bernard directly, something that instead he repeatedly does with Augustine, Thomas, Bonaventure, and many others. In this sense, many hypotheses are taken into consideration, including the idea that for the philosopher of Giarre, Bernard was not just a source, but a lens through which to look better and further awy. A silent relationship, therefore, kept away from the sight of most, that which Sciacca has allowed to grow between the folds of his writings. The two philosophers defy time and find themselves, or refind within a philosophical synchronicity that sees them as messengers of the same values. In this perhaps “imaginary” dialogue between a thinker who has been able to look beyond his century, and one who has never missed an opportunity to reflect and reflect through the past, we can find and enjoy the emotional tension of the human being who claims to reconnect with the Absolute. The article aims to provide the main conceptual coordinates in order to fully understand the state of the art of the most recent research in the field of neurobiology of interpersonal experience. The main purpose of this work is to analyze, at an anthropological, phenomenological and epistemological level, how the fundamental characteristics of the recognition of otherness and intercorporeity among human beings contribute to changing the image of nature in the light of a possible new relationship between living bodies, neurophysiological (...) systems and empathy. From this point of view, the hypothesis to investigate is that neurophenomenology, understood as a new evolutionary, multidimensional and autopoietic approach, is capable of probing the preconditions of the possible delineation of a phenomenology of intersubjectivity shaped by the neuroscientific turning point, represented by the discovery of mirror neurons. At this level, the neuroscientific data are interpreted according to a specific interdisciplinary perspective, thus trying to offer a possible unitary and integrated theoretical framework. (shrink)

Negation can be considered a shared social action that develops since early infancy with very basic acts of refusals or rejection. Inspired by an approach to the embodiment of concepts known as Multiple Representation Theories, the present paper explores negation as an embodied action that relies on both sensorimotor and linguistic/social information. Despite the different variants, MRT accounts share the basic ideas that both linguistic/social and sensorimotor information concur to the processes of concepts formation and representation and that the balance (...) between these components depends on the kind of concept, the context, or the performed task. In the present research we will apply the MRT framework for exploring negation in Italian sign language. The nature of negation in LIS has been explored in continuity with the co-speech gesture where negative elements are encoded through differentiated prosodic and gestural strategies across languages. Data have been collected in naturalistic settings that may allow a much wider understanding of negation both in speech and in spoken language with a semi-structured interview. Five LIS participants with age range 30–80 were recruited and interviewed with the aim of understanding the continuity between gesture and sign in negation. Results highlight that negation utterances mirror the functions of rejection, non-existence and denial that have been described in language acquisition both in deaf and hearing children. These different steps of acquisition of negation show a different balance between sensorimotor, linguistic and social information in the construction of negative meaning that the MRT is able to enlighten. Riassunto: Il presente contributo esamina il concetto di esperienza cosciente da una prospettiva epistemologica evoluzionista, ispirata ad un approccio naturalistico non riduzionista. Il lavoro si inserisce nel quadro concettuale delle ricerche nel campo della filosofia della mente, avanzando delle ipotesi circa i possibili processi che hanno determinato la comparsa e lo sviluppo nella nostra biosfera di una mente specificatamente umana sia dal punto di vista filogenetico che da quello ontogenetico. In quest’ottica, vengono rivisitati in chiave epigenetica alcuni tratti salienti della (...) teoria della coscienza di Edelman alla luce delle più recenti ricerche sulla misurazione dell’esperienza cosciente, condotte da Tononi e da altri studiosi nell’ambito della Integrated Information Theory. In un quadro siffatto, l’articolo analizza alcune criticità dell’approccio algoritmico della IIT, evidenziando la necessità di individuare nuove misure della complessità per quanto riguarda lo studio dei processi semantici di trasmissione dell’informazione biologica. The perspectives of the integrated information theory: This paper examines the concept of conscious experience from an evolutionary epistemological perspective, inspired by a non-reductionist naturalistic approach. Situated in the conceptual framework of philosophy of mind, it considers what processes could have led to the appearance and development of a specifically human mind in our biosphere, both from a phylogenetic and an ontogenetic point of view. Salient features of Edelman’s theory of consciousness are revisited from an epigenetic perspective, in light of some of the most recent measurements of conscious experience proposed by Tononi and other scholars in the field of Integrated Information Theory. The paper analyzes some criticalities in the algorithmic approach of the IIT, highlighting the need to identify new measures of complexity for the study of semantic processes of transmission of biological information. This chapter discusses the problem of time in the thought of Bergson, showing how the evolution of the concept of duration is conducive to new developments in the philosophy of intuition. Duration, which in the Essay connotes the experience of a non-measurable lived experience, while in Matter and Memory it assumes rhythms of different intensities to justify the relationship between perception and memory, as well as in Creative Evolution is judged as the fabric of reality itself, in Duration and Simultaneity (...) it is posited not only as a criterion to discern what is real and what is artificial, but also to justify the measurement of reality, that is to say, to restore the point of contact between time as duration and space, that seemed previously compromised by the loss of ontological consistency of extension as the dimension of the body. In this paper, then, Bergson, also by virtue of a controversial, detailed comparison with the theory of relativity, finally arrives to support the hypothesis of a single temporal dimension, curved and dynamic, where space comes to be outlined as the abstract tangent of time. It is at this level, then, that it becomes possible to examine the emergence of the concepts within the meanders of intuition as concepts imbued with meaning: here is the primary source of that continuous “added” element of new nuclei of creativity that characterizes the very logic of Bergsonian living where what, as ideal relationship, is time, as a real relationship, becomes life, i.e., the continuous opening of a register in which time is inscribed, becoming incarnate thus in the very nature of every organism. This paper treats a classical topic of scientific epistemology from a new point of view. It considers biology to be a science intermediate between physics and sociology, and the transition from physics to biology as proceeding upwards. As a consequence, any type of reductionism will be avoided. The foundation of sociology can now be viewed as an extension of physics and biology. Indeed social reality is built by means of constitutive rules that create those social facts that have been denominated (...) ‘institutional'. Having argued for the connection among values and norms and actions, the problem is that of justifying this connection. Can values and norms be reasons that explain action? Can reasons be understood as causes? In this paper the thesis is advocated that reasons are not sufficient for causally explaining actions. Taking up the classical analysis of ‘practical inference', I want to point out that, if from the reasons for action logically followed the action itself, the reasons would be sufficient causes of the action: indeed, this would eliminate free will. For this reason, we must examine the problem of free will. My conclusion is in favor of the position of B. Libet, who has demonstrated free will experimentally, and therefore the nondeterministic nature of the practical-inferential model. (shrink)

In human groups performing oscillatory tasks, it has been observed that the frequency of participants' oscillations reduces when compared to that acquired in solo. This experimental observation is not captured by the standard Kuramoto oscillators, often employed to model human synchronization. In this work, we aim at capturing this observed phenomenon by proposing three alternative modifications of the standard Kuramoto model that are based on three different biologically-relevant hypotheses underlying group synchronization. This article aims at elucidating the logic of Arist. SE 22, 178b36–179a10 and, in particular, of the sophism labelled "Third Man" discussed in it. I suggest that neither the sophistic Walking Man argument, proposed by ancient commentators, nor the Aristotelian Third Man of the , suggested by modern interpreters, can be identified with the fallacious argument Aristotle presents and solves in the passage. I propose an alternative reconstruction of the Third Man sophism and argue that an explanation of the lines (...) regarding the identity of Coriscus and Coriscus the musician (178b39–179a3) is indispensable for its correct understanding, since they hint at another sophism in some important aspects analogous. Finally, I show that two contradictions concerning spotted by scholars in the passage are only apparent and can be dissolved once the assumption that the anti-Platonic Third Man argument is at stake here is discarded, and once the passage is read in the light of its agonistic context. ObjectiveGonadotoxicity is considered one of the most distressing side effects of cancer treatment. Although fertility preservation can be a valid solution, it also involves a challenging process. A clear understanding of the features of women who decide to pursue fertility preservation after cancer diagnosis is missing. The purpose of the present study was therefore to analyze the personality profile of female patients referred to oncofertility prior to gonadotoxic treatment.MethodsFifty-two female cancer patients took part in the study. The Temperament and Character (...) Inventory-Revised, the Response Evaluation Measure-71, the Beck Depression Inventory, and the State-Trait Anxiety Inventory-Y Form were administered to examine personality characteristics, defense mechanisms, depression and anxiety symptoms.ResultsCompared with reference data of the Italian population, our sample reported significantly lower scores in Harm Avoidance and trait anxiety, and significantly higher levels of mature defense mechanisms. Most of the patients reported low scores in immature defense mechanisms, depression, and trait anxiety, and medium scores in state anxiety.ConclusionsOur findings suggest that these women display functional personality traits and defensive style, in association with low levels of depression and trait anxiety. These features may enable a proactive attitude to cancer and the ability to make long-term plans. This may favor psychological adjustment to cancer and a projection toward the future. (shrink)

Objective: Fertility loss due to cancer treatment can be a devastating experience for women and the couple. Undergoing fertility preservation can be a complex decision from both a medical and emotional point of view. The aim of the present study was to evaluate which socio-demographic and psychological factors predict a high motivation to undergo fertility preservation.Methods: Fifty-eight female cancer patients who accessed an Oncofertility Unit completed: a questionnaire to collect socio-demographic characteristics and the level of motivation, the Beck-Depression Inventory-II, the (...) State-Trait Anxiety Inventory-Y, and the Fertility Problem Inventory.Results: Almost half of the sample declared a high motivation. At multiple logistic regression analysis only the “Need for parenthood” subscale of the FPI predicted a high motivation. We alternatively evaluated as possible predictor the construct “Representations about the importance of parenthood” in place of the two separate subscales. At multiple logistic regression analysis, only this variable predicted a high motivation to undergo fertility preservation.Conclusion: The most important predictor of a high motivation to undergo fertility preservation is the individual desire for parenthood. This implies that, regardless of socio-demographic characteristics, any woman of childbearing age should receive an appropriate counseling about fertility preservation. (shrink)

While much of the knowledge requires the input of legal experts, we focus in this article on NLP applications that semi-automate essential time-consuming and lower-skill tasks—classifying legal documents, identifying cross-references and legislative amendments, linking legal terms to the most (...) relevant definitions, and extracting key elements of legal provisions to facilitate clarity and advanced search options. IntroductionDuring the COVID-19 pandemic, healthcare workers in Italy have been exposed to an unprecedented pressure and traumatic events. However, no direct comparison with the general population is available so far. The aim of this study is to detail mental health outcomes in healthcare workers compared to the general population.Methods24050 respondents completed an on-line questionnaire during the contagion peak, 21342 general population, 1295 second-line healthcare workers, and 1411 front-line healthcare workers. Depressive, anxious, post-traumatic symptoms and insomnia were assessed. Specific COVID-19 related (...) potential risk factors were also considered in healthcare workers.ResultsDepressive symptoms were more frequent in the general population and front-line healthcare workers compared to the second-line healthcare workers groups. Anxiety symptoms showed a prevalence of 21.25% in the general population, 18.05% for second-line healthcare workers, and 20.55% for front-line healthcare workers. Insomnia showed a prevalence of 7.82, 6.58, and 9.92% for the general population, second-line healthcare workers, and front-line healthcare workers, respectively. Compared to the general population, front-line healthcare workers had higher odds of endorsing total trauma-related symptoms. Both second-line healthcare workers and front-line healthcare workers had higher odds of endorsing core post-traumatic symptoms compared to the general population, while second-line healthcare workers had lower odds of endorsing negative affect and dissociative symptoms. Higher total traumatic symptom score was associated with being a front-line healthcare worker, having a colleague infected, hospitalized, or deceased, being a nurse, female gender, and younger age.ConclusionThis study suggests a significant psychological impact of the COVID-19 pandemic on the Italian general population and healthcare workers. Front-line healthcare workers represent a specific at-risk population for post-traumatic symptoms. These findings underline the importance of monitoring and intervention strategies. (shrink)

In this article, we publish the critical edition of Andalò di Negro’s De compositione astrolabii, with English translation and commentary. The mathematician and astronomer Andalò di Negro presumably redacted this treatise on the astrolabe in the 1330s, while residing at the court of King Robert of Naples. The present edition has three purposes: first, to make available a text missing from the previous compilations of works by Andalò di Negro; second, to revise a privately circulated edition of the text; and (...) third, to help disseminating one of the rare Latin texts presenting the principles of the stereographic projection which underlie the construction of the astrolabe.
